You have to fill in this form: Emigratie doorgeven. You have to upload a proof of deregistration in our country, date and such an we will contact you to settle it all.

Thanks. But the de-registration document will be given to me only 5 days before my departure. But you want 1 month notice for cancellation. How to deal with this? Will you consider notice period from today for a month? I want to cancel subscriptions from jan 31, 2021 onwards. 

Do you have other documents you can hand over that will tell you are leaving the country? Maybe something from your employer or something? Or call 0800-0402 and ask for the ‘opzeggingen’ department. They handle all the cancellations.
