The fastest and easiest way to look up your pukcode, is via MijnKPN. The page I linked to will show you the way. Please let me know if it worked out.

Hi @RachelVelvet there are several options.

The puk code is mentioned on the plastic card in which you received the sim card. You’ll find the puk code in MijnKPN and if that doesn’t work you can use this online form as well.
