I want to move to Poland permanently and I wanted to terminate the contract in kpn but deregistration paper is required And I don't have it yet because I will be deregistering on May 19, how can I arrange it in another way

The short answer is, you can't. When cancelling because of emigration we ask for proof of moving away. Once you have that, please use this form.
If your regular contractual time is over, you can cancel with a 1 month waiting period. To arrange this, you can call 0800-0402.
If your contract isn't over, I must ask you to please provide proof that you are moving out.
